A child must live in the Madison Station School zone and must be five years old by September 1, 2008.
New Students (K-5th) What to Bring:
1. Certified birth certificate
2. Social security card
3. MS immunization form 121
4. Warranty deed or lease agreement
5. Two (2) current utility bills (electric, water, gas, phone)
6. Copy of most recent Report Card
